Palworld Surgery Table, Implants and Dr. Brawn — How It Works

Verified on v1.0.5 · 23 Oct 20263 min read

In Palworld 1.0, the Pal Surgery Table (Tech Lv 38) grafts a passive skill onto a Pal for gold. It lists 54 passives: 19 need only gold, the rest need that passive's implant in your inventory. There are 61 implants, 40 permanent and 21 disposable, from the Arena, the Bounty Officer and the Ancient Relic Recycler.

This page explains how to use the table and where implants come from. The full list of every implant, its passive and its price is the implants catalogue.

How the Surgery Table works

The Pal Surgery Table is a base structure that unlocks at Tech Lv 38. Per palworld.wiki.gg it changes a Pal's gender or its passive skills, and it must be placed inside a Palbox base area. Gender needs a Pal Reverser; passives need gold, and for most of them the implant too.

Of the 54 passives the table lists, 19 are for gold alone and the rest only with their implant in your inventory. Follow the steps in order the first time you use it; the tick boxes stay in this browser.

  1. Unlock the table at Tech Lv 38

Open the Pal Surgery Table in the tech tree and spend its points.
  2. Place it inside a Palbox base area

The wiki states the structure must sit within a Palbox base area.
  3. Pick the passive you want

The table lists 54 passives. Check the implants catalogue for each one's effect and price.
  4. Get the implant if the passive needs one

19 passives are gold only. For the others, get the implant from one of the three sources below first.

  5. Pay the gold and confirm

The table asks 10,000 or 50,000 gold per passive (nothing for a disposable implant). The change is permanent unless modified again.

Where implants come from

There are 61 implants: 40 permanent ones you unlock once, and 21 disposable ones, each a one-time graft. The two kinds have different sources, summarised below; each row's catalogue page lists the exact implants.

SourceImplantsPrice or chance
Arena Merchant (permanent)750 Battle Tickets each

Start with the two shops

Every permanent implant with a source is sold: 7 at the Arena Merchant and 7 at the Bounty Officer. The catalogue links the merchant markers for each. Examples: Serenity, Infinite Stamina, Runner, Burly Body, Artisan, Musclehead.

What the surgery costs in gold

A graft costs 10,000 or 50,000 gold per passive, depending on the passive's rank, whether the passive is gold only or also needs a permanent implant. The table lists a price of 0 for the 21 passives that use a disposable implant.

  • 10,000 gold

per graft, one of the two prices

  • 50,000 gold

per graft, one of the two prices

Dr. Brawn: the random surgery

Dr. Brawn is a Pal doctor NPC. Per palworld.wiki.gg he can be found almost anywhere on the Palpagos Islands and appears randomly, and he will ask to experiment on a selected Pal "for the better or worse": accepting can lead to a random positive or negative stat change and passive skill change.

The wiki page is a stub, so this guide does not say how often he appears, whether he can be captured or what his stat change touches. Treat his offer as a gamble and do not spend it on the Pal you cannot replace.

Common mistakes

Using Dr. Brawn on your best Pal (the result can be negative), paying for a graft before checking the passive's effect in the catalogue, and chasing a disposable implant from the recycler at 0.051% a roll when a shop sells a permanent one for 50 Battle Tickets.

FAQ

How do I change a Pal's passive skills in Palworld?

Build the Pal Surgery Table (Tech Lv 38), place it in a Palbox base area, and graft the passive you want. 19 of the 54 passives cost gold only; for the rest you need that passive's implant, and the gold is 10,000 or 50,000 (none for a disposable implant).

Where do I get implants in Palworld?

Permanent implants are sold by the Arena Merchant (50 Battle Tickets) and the PIDF Bounty Officer (15 Successful Bounty Tokens). Disposable ones come from the Ancient Relic Recycler and a few Ancient Ruin chests.

Are implants consumed when you use them?

Permanent implants are not: palworld.wiki.gg says each one only needs to be obtained once per skill. The 21 disposable implants are single use by name.

Who is Dr. Brawn in Palworld?

A Pal doctor who appears randomly almost anywhere on the Palpagos Islands and offers to experiment on a Pal, with a random positive or negative stat and passive-skill change (palworld.wiki.gg).
